- You must fill out and turn in a new W-9 each year.
- Direct deposit forms are required if you want to get paid! You do not need to submit this form more than once unless your banking information changes.
- Partnership banking authorization forms are only necessary if you have a partnership with another owner and you do not have a business entity. If you previously received paper checks written to both partners, you will need this form.
- State licensing forms should be emailed to firstname.lastname@example.org
